Spirit Group Request Form

Please fill out this form completely. Completion of this form does not guarantee an appearance. The following information is a guide to assist you in having Sammy C. Seahawk (the official mascot of UNCW Athletics), the UNCW Cheerleaders or the UNCW Dance Team at your upcoming event. All requests must be submitted in this format and will be considered on an individual basis if they are sent to the attention of Mike Vaccaro four weeks prior to the event. Appearances are generally scheduled one month prior to the event date and are subject to UNCW Athletics approval and availability. Secure parking and a dressing area should be provided for Sammy C. Hawk. In some cases, it may be necessary to provide water. As of July 1, 2011, events will be charged a fee of $25 for the first hour and $15 for each additional hour (non-profit groups are exempt from all fees).
